Diagnosis of Arrhythmogenic Right Ventricular Cardiomyopathy (ARVC)

Arrhythmogenic Right Ventricular Cardiomyopathy (ARVC) is a genetic heart muscle disorder that primarily affects the right ventricle. The 2010 revised task force established diagnostic criteria, using six factors to classify cases as definite, borderline, or probable based on major and minor criteria.

Diagnostic Criteria

  1. Imaging: Identifies regional or global RV dysfunction using CMR, TTE, or angiography.
  2. Biopsy: Shows fibrous tissue replacement in the RV myocardium.
  3. ECG Repolarization and Depolarization Abnormalities: Characterized by Epsilon waves and T-wave inversions.
  4. Arrhythmias: NSVT/VT with specific ECG patterns.
  5. Family History: Includes first-degree relatives with confirmed ARVC or known pathogenic mutations.

Major Diagnostic Tools

  • CMR & TTE: Imaging techniques to assess RV size, structure, and function.
  • ECG Findings: Epsilon waves in V1-V3 and T-wave inversions.
  • Biopsy Analysis: Morphometric assessment of residual myocytes and fibrous tissue.
